Abstract

Objective: Osteoarthritis is a global health problem that affects people of different age groups worldwide. It is known that this significant public health problem is more common in at-risk communities and disadvantaged groups within these communities. This study aimed to evaluate the quality of life (QoL) among elderly adults diagnosed with osteoarthritis (OA) in Nasiriya, Iraq, and to explore the associations between QoL and key sociodemographic and clinical variables within a public health context. Method: A descriptive cross-sectional design was employed with a sample of 150 individuals aged 65 years and elderly who presented to the Orthopedics and Traumatology outpatient clinic of Al-Hussein Training Hospital between March and September 2022. Data were collected using a structured personal information form and the Osteoarthritis Quality of Life Scale (OAQoLS). Statistical analysis included the Mann–Whitney U and Kruskal–Wallis tests for group comparisons, and Kendall’s Tau-b correlation to assess relationships between variables. A significance level of p<0.05 was applied. Results: The mean OAQoLS score was 33.2±6.8, indicating a moderate level of life quality. A significant negative correlation was found between age and OAQoLS scores (r = –0.539; p<0.0001). Marital status, education level, cohabitation arrangement, presence of comorbidities, and disease duration significantly influenced OAQoLS scores, whereas gender, income status, and smoking did not yield statistically significant differences. Notably, individuals living alone exhibited the lowest OAQoLS scores, suggesting a potential link between isolation and disease burden. Conclusion: The findings highlight age and selected sociodemographic factors as significant determinants of OAQoLS in elderly adults with OA. These findings highlight the critical need for early identification and scalable public health interventions to reduce the burden of osteoarthritis and improve quality of life among elderly adults. Public health efforts should prioritize community-based screening programs, patient education, and accessible rehabilitation services. Moreover, integrating osteoarthritis prevention into national aging policies can mitigate the burden of disability and improve overall well-being in elderly populations. Culturally tailored interventions and age-friendly urban planning may further contribute to enhancing quality of life among individuals affected by osteoarthritis in Iraq and similar cultural contexts.
