Abstract

Occupational accidents in Turkey vary across sectors and causes, exhibiting a wide distribution. In particular, machinery-related accidents continue to pose critical risks to occupational health and safety. This study adopts Reason’s Swiss Cheese Model to conduct an in-depth analysis of the direct and indirect causes behind occupational accidents. The model emphasizes that accidents do not arise solely from individual errors but rather from successive weaknesses across interdependent defense layers. Within this framework, machinery-related occupational accidents are examined and categorized into unsafe behaviors and unsafe conditions. These two components are conceptualized as distinct “layers” in the study, each associated with specific types of human error. This approach enables the identification of latent system vulnerabilities and highlights the significant role of human factors in accident causation. The dataset for this research is derived from Turkish Supreme Court (Yargıtay) case decisions concerning occupational accidents. These legal decisions offer a unique data source as they provide detailed insights into incident circumstances, fault distributions, types of negligence, and identified safety deficiencies. Accordingly, legal documents serve not only judicial purposes but also a valuable source for proactive occupational health and safety analyses. In this study, 620 Supreme Court decisions were screened, and 124 machinery-related cut–crush–ejection/splash cases meeting predefined criteria were included in the analysis. Methodologically, data were collected from Supreme Court decisions and analyzed through content analysis. Subsequently, critical layers contributing to accidents were identified using the Random Forest algorithm. Finally, THERP and PSF values were applied to these layers to calculate human error probabilities and final reliability scores. The analysis quantitatively identified the most critical human factors affecting accidents, and the error probabilities associated with these factors were calculated. Based on these findings, a layered risk assessment framework was developed to enable the proactive prevention of occupational accidents.
