Abstract

Creatinine kinase CK is an enzyme that controls energy transfer in the cells. Serum CK levels is increased in some instances including muscle diseases, trauma, exercise, rhabdomyolysis, hypoxia, status epilepticus, acute psycho- sis and aggressive behaviours. An 8-year-old boy lost and confined for one day in a mountainous terrain was admit- ted to our hospital His medical and family history were unremarkable. Vital signs were stable on physical examina- tion. Any abnormal finding apart from lethargy was not detected. In laboratory examination, glucosuria was found. Ketonemia was not observed. Hemoglobin, white blood cell and thrombocyte counts were 13.3 g/dL, 18.210 mm3 and 520.000 mm3, respectively. Serum electrolytes, calcium and phosphorus levels were within normal limits. Blood glu- cose level, CK, creatinine, aspartate aminotransferase and alanine aminotransferase levels were 183 mg/dL, 4779 U/L, 0.57 mg/dL, 169 U/L and 33 U/L, respectively. After a week, a control laboratory examination was performed. Blood glucose level, CK, aspartate aminotransferase and alanine aminotransferase levels were then 74 mg/dL, 118 U/L, 32 U/L and 10 U/L, respectively. In our case, we emphasize that high levels of serum CK and glucose may be related to stress, psychologic trauma, starvation and quivering

Fizik muayenesinde, vital bulgu- ları stabil idi. Halsizliği dışında anormal bir bulgu saptan- madı. Laboratuvar incelemesinde, idrarda glukozüri vardı. Kan ketonu negatifti. Tam kan sayımında hemoglobin 13,3 g/dL, beyaz küre sayısı 18,210/mm3, trombosit sayısı 520,000 mm3 idi. Serum elektrolitleri, kalsiyum ve fosfor değerleri normal sınırlarda idi. Kan şekeri 183 mg/dL, CK 4779 U/L, kreatinin 0.57 mg/dL, aspartat aminotransferaz 169 U/L ve alanin aminotransferaz 33 U/L idi. Bir hafta sonra kontrol kan şekeri 74 mg/dL, CK 118 U/L, aspartat aminotransferaz 32 U/L ve alanin aminotransferaz 10 U/L geldi. Vakamızda geçici serum CK ve glukoz yüksekliğinin stres, psikolojik travma, açlık ve soğuk havada titremeye bağlı olduğunu vurgulamak isteriz
