Abstract

Background: Oral epithelial dysplasia (OED) represents a potentially malignant condition with potential progression to oral squamous cell carcinoma (OSCC). However, histopathological diagnosis based on incisional biopsy may underestimate lesion severity due to sampling limitations, particularly in large or heterogeneous lesions. Case Presentation: An 89-year-old female presented with a painless, exophytic papillomatous lesion in the anterior maxilla. Clinical and radiological findings raised suspicion of malignancy. Initial incisional biopsy revealed mild epithelial dysplasia. Due to persistent clinicopathological discordance, an excisional biopsy was performed, which demonstrated papillary squamous cell carcinoma (PSCC), well-differentiated (G1), with a depth of invasion of 3 mm and tumor thickness of 5 mm, without lymphovascular or perineural invasion. Surgical margins were positive. Conclusion: This case shows the limitations of incisional biopsy in accurately representing heterogeneous oral lesions. When clinical and histopathological findings are not matching, further sampling or complete excision is essential. Careful clinicopathological correlation remains critical for early and accurate diagnosis, preventing delays in the management of oral malignancies.
