Abstract

Objectives: To present the clinical characteristics and treatment outcomes of retinal detachment secondary to pseudophakia and aphakia. Methods: Data of 62 patients who underwent conventional retinal detachment surgery or pars plana vitrectomy secondary to pseudophakia or aphakia between 1992-2000 at our Retina Unit were evaluated. Risk factors such as peroperative disintegration of posterior capsule, vitreous loss and postoperative Nd:YAG capsulotomy were evaluated. The patients with high myopia, history of trauma and peripheral retinal degeneration were excluded from the study. Results: Mean age of 62 patients was 61,5 years (range: 36 - 89). Thirty patients (48,4%) were aphakic and the remainder (51,6%) were pseudophakic. Of pseudophakic patients 25 (78,1%) had intraocular lens in the posterior chamber and seven (21,9%) had in the anterior chamber. The time period between the cataract extraction and development of retinal detachment was less than one year in 23 patients (37,2%). The number of patients presenting with reti-nal detachment symptoms in the first week was 14 (22,6%). Forty-two patients (67,7%) had single and 14 (22,6%) had multiple tears; no tear was detected in 6 patients (9,7%). At least stage B proliferative vitreoretinopathy was present in 38 of the patients (61,3%). In 37 cases the macula was involved (59,7%). Postoperative follow up period varied between 6 month and 5 years. It was found out that anatomic improvement was achieved in 44 eyes (70,9%) after the first operation. The visual acuity was increased in forty-three eyes (69,4%), did not change in 10 eyes (16,1%) and decreased in 9 eyes (14,5%). After reoperations final anatomic and functional success rates were found to be 83,9% and 72,6% respectively. Comment: Anatomical and functional success in patients with pseudophakic or aphakic RD depends on the duration and the extent of retinal detachment as well as the presence of macular involvement and proliferative vitreoretinopathy at presentation. The longer the interval between onset of retinal detachment and presentation to the hospital, the lesser is the anatomical and functional success.

Olguların 18'ine (%29.0) uygulanan mükerrer cerrahi sonrasında, anatomik başarının %83.9'a çıktığı ve fonksiyonel başarının %72.6 oranında sağlandığı görüldü. Tartışma: Psödofak ve afak hastalar regmatojen retina dekolmanlarını geç farkederek geç başvurmaktadırlar. Hastaların geç başvurması, başvuru anında makula tutulumunun mevcudiyeti, total retina dekolmanı ve proliferatif vitreoretinopati varlığı anatomik ve görsel sonuçlan olumsuz etkilemektedir.
