Abstract

Objective: To evaluate the efficacy, results and complica- tions of clear lensectomy and posterior chamber intraocular lens (IOL) implantation in high myopic eyes. Study design: Prospective clinical trial Materials and methods: Twenty-five eyes of 13 patients who have myopia of 12 diopters or more were included in the study. Mean age was 42.12 years. Routine ophthalmo- logic and peripheral retina examinations were done. Patients with peripheric degenerations were treated with argon laser. In intra ocular lens (IOL) calculation SRK/T formula have been used. All patients had temporal limbal incision phacoemulsification surgery. Mean follow up peri- od was 16.13 months. Results: Preoperative refractions were mean -16.13 D. Mean axial length was 30.12 mm. Uncorrected visual acuities were improved in all eyes. Best corrected visual acuities were improved in 24 eyes (96%). Thirtysix percent of patients had 1 line and 60% of patients had 2 lines of improvement in visual acuity. Eighty four percent of eyes had within 1 diopter and 96% of eyes had within 2 diopters of refractive error. No serious intraoperative complications. Retinal detachment did not occur in any patient after sur- gery. Four of the eyes have had YAG laser capsulotomy because of posterior capsule opasification. Conclusion: Clear lensectomy with posterior chamber IOL implantation is an effective and safe surgical method of refractive correction in high myopic eyes. The develop- ment of modern micro surgery, viscoelastic materials, and improvement in surgical knowledge and skills achieved with the development of new posterior chamber IOLs are positive factors to be considered for surgical correction of high myopia.

Ortalama aksiyal uzunluk 30.12 mm idi. Düzeltilmemiş görme keskinlikleri bütün gözlerde artmıştı. Düzeltilmiş görme keskinliği 24 gözde (%96) artmıştı. Bunların %36 sında 1 sıra, %60’ında 2 sıra artış olmuştu. Refraktif hata %84 gözde 1.0 dioptri içinde, %96 gözde 2.0 dioptri içindeydi. İntraoperatif komplikasyon oluşmadı. Cerrahi sonrası izlem süresinde hiçbir hastada retina dekolmanı görülmedi. Arka kapsül kesifliği gelişen 4 göze Yag laser kapsülotomi yapıldı. Sonuç: Arka kamara göziçi lens implantasyonlu şeffaf lens çıkarılması ameliyatı yüksek miyop gözlerin refraktif düzeltilmesi için etkili ve uygun bir yöntemdir. Modern mikrocerrahinin ve viskoelastik maddelerin gelişmesi, cerrahi bilgi ve tecrübelerimizin artması, yeni göziçi lenslerin gelişmesi yüksek miyopinin cerrahi düzeltilmesi için pozitif faktörlerdir.
