Abstract

Acute cholecystitis is one of the most common diseases of the biliary tract. Spontaneous regression of the inflammation is observed in about 90% of cases and remaining 10% of cases progress to necrosis, than to perforation. Virulent bacteria is present in the bile of 50% of patients with acute cholecystitis, which is similar to intestinal flora. In order to investigate the necessity of anti-anaerobic antibio-prophylaxis in the treatment of acute cholecystitis, we analyzed the bile specimens of 25 patients who underwent cholecystectomy due to severely acute cholecystitis. Results are evaluated with related literature. In 76% of patients (19 patients), bacteria have grown in the bile. Aerobic bacteria was found in all of 19 cases, while anaerobic species were present in 6 patients in addition to aerobic bacteria. Most frequented aerobic type was E.coli, and anaerobic type Clostridium sp. Five of 6 patients carrying anaerobic bacteria, are diabetic patients. In conclusion, we are convinced that anti-anaerobic antibiotics should take place in the prophylaxis and treatment of acute cholecystitis, especially in elderly and diabetic patients.

Anaerob bakterilerin ürediği 6 hastanın 5'i Diabetes mellitus'lu hastalardır. Çalışmadan çıkan sonuç, akut kolesistitlerde, özellikle yaşlı ve diyabetli hastalarda, anaerob suşlara etkili antibiyotiklerin de profilakside yer alması gerektiği yönündedir.
