Abstract

Objective: Moderate-to-large pericardial effusion in children is a potentially life-threatening condition that requires prompt diagnosis and timely intervention. However, data focusing specifically on children requiring invasive management remain limited. This study aimed to evaluate the etiological spectrum, clinical characteristics, management, and outcomes of children undergoing invasive intervention for moderate-to-large pericardial effusion. Methods: This retrospective single-center study included children (0–18 years) who underwent pericardiocentesis or surgical drainage for moderate-to-large pericardial effusion between January 2012 and December 2024. Demographic, clinical, laboratory, echocardiographic, procedural, and outcome data were reviewed. Results: Thirty-eight children underwent invasive intervention during the 13-year study period. Infectious diseases were the most common etiology (36.8%), followed by idiopathic, rheumatologic, and malignant causes. Respiratory symptoms were the predominant presenting manifestation (68.4%), and cardiac tamponade was present in 31.6% of patients. Pericardiocentesis was the initial treatment in 30 patients, of whom 15 (50.0%) required no further invasive intervention, while 19 (63.3%) did not require surgical drainage. Eleven patients (36.7%) ultimately required surgical drainage during follow-up. Procedure-related complications were uncommon; hemopericardium requiring emergency surgery occurred in two patients (5.3%). In-hospital mortality was 13.2% and was attributable to severe underlying diseases rather than procedural complications. Conclusion: Infectious diseases remain the leading cause of clinically significant pediatric pericardial effusion requiring invasive intervention. Echocardiography-guided pericardiocentesis is an effective initial treatment for most patients; however, recurrent, loculated, or complex effusions frequently require surgical management. Early recognition, accurate etiological diagnosis, and individualized multidisciplinary management are essential to optimize outcomes in children requiring invasive treatment.
