Abstract

Objective: We evaluated the effects of preoperative percutaneous biliary drainage on perioperative mortality and morbidity in patients with malign biliary obstructions. Methods: A total of 32 patients with ages ranging from 45 through to 72 years were carried out preoperative percutaneous biliary drainage (PBD) under the guidance of US and x-ray fluoroscopy. External and internal-external biliary drainages were attempted in 14 and 18 patients respectively.Bilirubin levels were recorded before and after PBD. The patients who had got the better of general condition and bilirubin level lower than 10 mg/dl were referred to the surgery. Results: Preoperative PBD was performed in 32 patients with obstructive jaundice, diagnosing 13 pancreatic carcinoma, 7 klatskin tumor, 2 metastatic mass, 2 gall-bladder carcinoma, and 4 peri-ampullar carcinoma. Serum bilirubin levels were 17.8 4 mg/dl and 8.1 3 mg/dl before and after PBD respectively. Preoperative biliary drainage improves the liver function and general condition, so that major operations can be safely performed. Only 2 major complications were observed (sclerosan cholangitis and sepsis) in two patients. There were no complications in the remaining patients. Perioperative mortality rates was 12.5%. Conclusion: Preoperative biliary drainage improved hepatic function while diminishing surgical morbidity and mortality. PBD improved the prognosis and should be used routenly in patients with malign biliary obstructions.
