Abstract

Aim: In the setting of isolated aortic stenosis, left ventricular strain pattern, defined as more than 2 mm of ST depression, suggests severe left ventricular hypertrophy. This study has examined the relation of electrocardiographic left ventricular strain pattern with symptoms and transaortic gradient noted during clinical follow-up. Materials and Methods: Patients with hypertension, coronary artery disease, moderate to severe aortic or mitral insuffiency, left ventricular systolic dysfunction, bundle branch block and individuals taking drugs capable of causing ST segment changes were excluaded from study. A total of 46 patients who fulfilled the entry criteria were enrolled in the study. All subjects underwent coronary angiography as well as left heart catheterization, during which trans-aortic valvular gradients were obtained. The study were divided into two groups on the basis of having or lacking electrocardiographic left ventricular strain pattern. Both groups were then assessed in terms of clinical symptoms (angina, syncope) and measured transaortic gradients. Results: When a threshold valve of gradient 50 mmHg was selected for evaluation, a strong correlation was detected between the electrocardiographic left ventricular strain pattern and catheter evaluated peak transaortic gradient (p<0.001). Hence, the peak transaortic systolic gradients were 88±30 mmHg and 47±19 mmHg in subjects with and without left ventricular strain pattern, respectively. The electrocardiographic left ventricular strain pattern was found to predict a peak gradient ³50 mmHg with a sensitivity of 92.3% and a specificity of 67.9%. A significant correlation was also detected between sympthoms of angina or syncope and the gradient (p<0.001). In the setting of aortic stenosis, presence of angina or syncope have been associated with an 8.3 fold increased risk of peak systolic gradient being ³50 mmHg (95% CI 1.8-38). Conclusıon: There is a significant relationship between the electrocardiographic left ventricular strain pattern and the transaortic peak systolic gradient and symptoms (angina or syncope). This particular correlation may be helpful to the clinician in the management of aortic stenosis patients.
