Abstract

AIM: Pulmonary hypertension (PHT) is a primary cardiac complication of chronic obstructive pulmonary disease (COPD) and the importance of its early diagnosis and therapy is still well known. Many mechanisms are responsible for the development secondary PHT. Renin- angiotensin-aldosteron system is one of the major components and the pharmacologic manipulation angiotensin II (AT-II) was shown to lower pulmonary vasoconstriction due to hypoxia. In this study we evaluated by echocardiography the effect of an AT-II receptor antagonist, Losartan, on the mean pulmonary arterial pressure (MPAP) in patients with secondary PHT due to COPD. MATERIAL AND METHOD: Forty-five COPD patients with more than 5 years follow-up and no additional comorbid entities were initially assessed by transthoracic echocardiography (TTE) proved to have PHT. After receiving written consent the patients were randomized to receive either Losartan 50mg/day p.o. (22 patients,male/female: 12/10) or no medication (control group, 23 patients,male/female: 12/11). At the third months of the randomization echocardiographic and clinical evaluations were repeated. MPAP was calculated by Mahan's formula using a 3.5 MHz TTE probe. RESULTS: In the Losartan group a significant increase in pulmonary acceleration time (PAT) was observed (p=0.005, initial PAT: 85.4±14.1 msec, control PAT 115.0±13.3 msec, 95% CI -35.1-23.9). The control group showed no significant change at the same period. In the Losartan group we observed a decrease of MPAP from 40.5±6.3 mmHg to 27.2±6.0 mmHg (p=0.005, 95% CI: 10.7-15.6) with no considerable side effects. Three months' treatment period with Losartan resulted in a statistically significant drop of MPAP (13.2±5.6 mmHg) in comparison to the control group, which showed no change (p
