Abstract

Aim: Electrocardiographic indices like maximum P-wave duration (Pmax) and P-wave dispersion (PDD) can be used to detect patients with atrial conduction disorders, myocardial ischemia and those at risk for atrial fibrillation. Considering the diurnal variation of ischemia in patients with significant coronary lesions this study was designed to investigate the diurnal variation of their eventual atrial conduction abnormalities. Material and Method: Thirty-one male and 17 female patients with typical angina were grouped according to their coronary angiography results into Group I - having 70% or more lesions in at least one of their coronary arteries (n=28), and Group II - with normal coronary arteries (n=20). The difference between the longest and shortest P-wave durations on the 12-lead electrocardiography is designated as P-wave Dispersion (PDD) (PDD=Pmax-Pmin). The diurnal Pmax, Pmin, PDD values were compared between and within the groups. Results: The morning Pmax value of Group I (112±1ms) was significantly higher when compared to Group II (102±1ms) (p<0.001). The morning PDD value of Group I (54±9ms) was significantly higher than that of Group II (48±1ms) (p<0.05). The morning Pmax value of Group I (112±1ms) was significantly higher than its afternoon and night Pmax values (102±9ms, 102±1ms, respectively, p<0.001), whereas there was no significant difference between the Pmax values within Group II (p>0.05). The morning PDD value of group I was higher than its afternoon and night PDD values (54±9ms, 40±10ms, and 43±9ms, respectively, p<0.001). Conclusion: The higher morning Pmax and PDD values in Group I are important with regard to timing and treatment of atrial conduction disorders in myocardial ischaemia.

Bulgular: Grup I'in sabahki Pmax değeri (112±1ms), Grup II'nin Pmax değerine (102±1ms) göre anlamlı olarak yüksekti (p 0.05) bulundu.
