Abstract

Benign transient hyperphosphatasemia, which is usually seen in children aged between 2 months and 2 years and detected during routine laboratory evaluation for some unrelated complaint, is a clinical condition characterized by high serum alkaline phosphatase (ALP) levels with 3-50 fold above the upper normal values for age. The cause is unknown. Generally, spontaneous decrease in ALP levels to normal values is determined within 4-6 months. Differential diagnosis should be made with chronic idiopathic hyperphosphatasia which is a rare generalised skeletal dysplasia characterised by diffuse, symmetrical and progressive bone deformity with high levels of both serum ALP and urinary excretion of hydroxyproline. Here, we present a two year old girl who was admitted to our clinic because of walking and speech delay and had increased serum ALP levels (4950 mcg/L) in laboratory evaluation and diagnosed as benign transient hyperphosphatasia with a decrease in serum ALP levels spontaneously to normal values within 1 month.
