Abstract
Introduction: Balance and gait impairments are common in individuals with Parkinson’s disease (PD). Impaired upright posture, reduced axial and spinal mobility, and diminished cervical and trunk proprioception negatively affect postural adaptation and mobility. These functional limitations often lead to decreased motivation, social withdrawal, and a more isolated lifestyle. This study aims to examine the effects of group exercise on disease severity, functional balance, and proprioceptive sense in a patient with PD. Case: A 78-year-old patient diagnosed with PD at stage 2.5 on the Modified Hoehn and Yahr Scale was included. Socio-demographic data were recorded. The patient participated in an 8-week group exercise program supervised by a physiotherapist, alongside five other individuals with similar disease severity. Disease severity was evaluated using the Unified Parkinson’s Disease Rating Scale (UPDRS). Balance was assessed through the Tandem Stance Test (TST), Timed Up and Go Test (TUG), and Functional Reach Test (FRT). Cervical and trunk proprioception were measured using the joint position reproduction test. Assessments were conducted pre-treatment (PT) and post-treatment (PoT). No difference was observed in TST PT/PoT respectively (0s/0s). However, improvements were found in other outcomes: UPDRS PT/ PoT respectively (35/16), TUG PT/PoT respectively (16.86s/14.59s), FRT PT/PoT respectively (23.3 cm/27 cm), cervical proprioception PT/ PoT respectively (10°/3°), and trunk proprioception PT/PoT respectively (7.6°/3.3°). Conclusion: Group-based exercise showed positive effects on disease severity, balance, and proprioception in a patient with PD. It may also reduce social isolation and enhance motivation. These findings suggest group exercise could be a beneficial component of physiotherapy in PD management.
