Abstract

Prostate cancer is the second most common cancer among men worldwide. The Gleason Score (GS) system is a crucial prognostic parameter in prostate cancer management. The cribriform pattern is a morphological subtype associated with aggressive disease behaviour in prostatic adenocarcinomas. Since 2014, all tumors with cribriform patterns have been classified as Gleason Score 4, and reporting their presence has been recommended. In this study, number of 101 Gleason Score 7 (Grade Groups 2 and 3) prostate adenocarcinoma cases who underwent robot-assisted radical prostatectomy between 2012 and 2019 at Ankara Atatürk Training and Research Hospital and Ankara Bilkent City Hospital were analyzed. Significant associations were found between the cribriform pattern percentage and multifocality, perineural invasion, positive surgical margins, and seminal vesicle infiltration. Additionally, an inverse relationship was observed between the cribriform pattern percentage and lymphovascular invasion. Although no significant relationship was found between postoperative PSA levels and cribriform morphology. The findings suggest that the cribriform pattern percentage should be considered a prognostic parameter in prostate cancer management. However, further large-scale and long-term studies are required.
