Abstract

Aim: Incidental findings refer to abnormalities that do not show clinical signs but are detected in images taken for other purposes. Some of the findings require therapeutic interventions and can prevent future complications. Cone beam computed tomography (CBCT) offers an effective evaluation of anatomical variations due to its high resolution and ability to provide three-dimensional images. The aim of this study was to evaluate the presence of incidental findings on CBCT and the need for referral. Material and Method: 200 CBCT images with a large FOV were retrospectively evaluated. Findings were categorized as airways, nasal, soft tissue calcifications, temporomandibular joint (TMJ), dental, jaw bones and vertebrae. Patients were divided into 3 age groups: under 20 years, between 20-50 years and over 50 years. Patient age, gender, indication for CBCT, ordering department, presence of incidental findings and need for referral were recorded. Descriptive statistical parameters were calculated as mean values and percentages. Results: A total of 1470 incidental findings were detected in 200 KIBT scans. The most common incidental finding was infraorbital canal protrusion (10.47%), followed by septum deviation (9.18%), nasal turbinate hypertrophy (8.57%), TMJ flattening (6.46%), vertebral sclerosis (6.33%), osteosclerosis (6.05%), periodontal loss (5.37%) and periodontal ligament enlargement (5.37%). Of the incidental findings, 135 (9.2%) required referral, 984 (66.9%) required follow-up and 351 (23.9%) required neither. Conclusion: This study confirms the high frequency of incidental findings in CBCT scans. In CBCT examinations, not only the targeted anatomical region but also the entire volume obtained by imaging should be systematically evaluated and reported. This approach is important for the detection of incidental findings in the cranial region, cervical vertebrae, nasal structures and soft tissues; thus, clinically significant lesions can be prevented from being missed.

Hastanın yaşı, cinsiyeti, KIBT endikasyonu, Field of View (FOV), istem yapan bölüm, tesadüfi bulgu varlığı ve yönlendirme ihtiyacı kaydedildi. Ortalama değer ve yüzde değerleri şeklinde tanımlayıcı istatistik parametreleri hesaplandı. Bulgular: 200 KIBT taramasında toplam 1470 tesadüfi bulgu tespit edildi. En yaygın tesadüfi bulgu infraorbital kanal protrüzyonu (%10,47) olup, bunu septum deviasyonu (%9,18), nazal konka hipertrofisi (%8,57), TME düzleşme (%6,46), vertebral skleroz (%6,33), osteoskleroz (%6,05), periodontal kayıp (%5,37) ve periodontal ligament genişlemesi (%5,37) takip etmiştir. Tesadüfi bulguların 135’i (%9,2) yönlendirme gerektirirken, 984’ü (%66,9) takip gerektirmiş, 351’i (%23,9) ise her ikisini de gerektirmemiştir.
