Abstract

Maxillary sinus elevation (SE) is a widely adopted surgical technique designed to address vertical bone loss resulting from alveolar crest resorption and maxillary sinus pneumatization following tooth extraction. Originally introduced to facilitate implant placement in the posterior maxilla with insufficient residual bone height, this procedure has undergone various modifications and is now routinely implemented in clinical practice. Two primary approaches are commonly used: lateral (open) and transcrestal (closed), with the method selection based on patient-specific anatomical features, available bone height, and implant treatment planning. Transcrestal sinus elevation is a minimally invasive alternative, particularly suitable for cases with moderate bone height. Due to its advantages—such as reduced morbidity, shorter operative time, and faster postoperative recovery—it has become increasingly favored by clinicians. Compared to the lateral approach, the transcrestal technique is less invasive and provides enhanced patient comfort. Nevertheless, the success of transcrestal sinüs elevation largely depends on the careful manipulation of the Schneiderian membrane, accurate assessment of the residual bone, and effective use of surgical tools. In recent years, the development of novel transcretal sinüs elevation techniques and instruments has further enhanced the predictability and success of the procedure. Comprehensive case-specific clinical and radiographic evaluations remain essential to minimize complications and optimize outcomes. This review summarizes current approaches to transcrestal sinus elevation techniques, highlighting the clinical significance of appropriate case selection and technical planning for achieving reliable and predictable implant success in the posterior maxilla.
