Abstract

In addiction treatment, traditional and high-threshold service models remain inadequate in reaching marginalized groups experiencing social exclusion and stigmatization. This study aims to examine the role and functionality of street work in combating drug addiction through the model of Fondazione Villa Maraini operating in Italy. Designed as a qualitative case study, data were collected through participant observation and document analysis techniques, and subsequently analyzed using thematic analysis. The findings indicate that the examined model stratifies interventions into very low, low, and high-threshold levels within the framework of a "continuum of care" philosophy. Within this structure, street work functions as a proactive initial point of contact that does not impose immediate change or absolute abstinence on individuals, but rather prioritizes harm reduction and the prevention of overdose deaths. Street units establish a trust-based bridge between the institution and marginalized clients, assuming a structural role in combating stigmatization. Consequently, it is essential to move beyond exclusively high-threshold and medical treatment-oriented approaches in addiction care to develop flexible models that prioritize outreach and accessibility. Drawing on the Fondazione Villa Maraini example, this study presents policy recommendations for Türkiye’s ecosystem for combating addiction, encompassing the establishment of low-threshold drop-in centers, the institutionalization of street work, and the integration of ex-users into professional processes.
