Abstract

A pacemaker is a device that regulates the heart rhythm by electrically stimulating the myocardium when the heart cannot generate sufficient electrical impulses or when the heart rate decreases due to conduction disorders. While primarily used in cats and dogs for symptomatic bradycardia, pacemakers are also indicated in pathologies such as sick sinus syndrome, grade II-III atrioventricular block (AVB), and permanent atrial arrest. A pacemaker system consists of a generator, battery, electronic circuits, and electrodes implanted in the heart. Modern devices can sense the heart's natural activity, provide stimulation, when necessary, adapt the heart rate, and offer significant advantages in clinical monitoring thanks to their programmable features. This review summarizes the details of pacemaker implantation in the treatment of pathologies related to the electrical conduction of the heart in small animal practice.

Özet

Kalp pili (pacemaker), kalbin yeterli elektriksel uyarı oluşturamadığı veya kalpteki iletim bozukluğu nedeniyle kalp hızının düştüğü durumlarda miyokardı elektriksel olarak uyararak ritmi düzenleyen bir cihazdır. Kedi ve köpeklerde özellikle semptomatik bradikardilerde uygulanmakla birlikte hasta sinüs sendromu, II.-III. derece atriyoventriküler blokaj (AVB) ve kalıcı atriyal duraklama gibi patolojilerde de kalp pili endike olmaktadır. Pacemaker sistemi; jeneratör, batarya, elektronik devreler ve kalbe yerleştirilen elektrotlardan oluşur. Modern cihazlar kalbin doğal aktivitesini algılayabilir, gerektiğinde uyarı verebilir, hız adaptasyonu sağlayabilir ve programlanabilir özellikleri sayesinde klinik takipte önemli avantajlar sunar. Bu derlemede küçük hayvan pratiğinde kalbin elektriksel iletimine ait patolojilerin tedavisinde kalp pili uygulamasının detayları özetlenerek aktarılmıştır.
