Abstract

Introduction: Surgical and adjuvant treatments for breast cancer cause significant changes in upper extremity function. Objective: This study aimed to investigate the effects of regularly performed reformer pilates exercises on shoulder range of motion (ROM), pain, upper extremity function, and body image in a case diagnosed with breast cancer. Methods: The case, a 39-year-old individual diagnosed with breast cancer, underwent total mastectomy surgery. In the postoperative period, 8 cycles of chemotherapy and 25 sessions of radiotherapy were administered, and the patient has been followed with hormone therapy (tamoxifen). Seven months after surgery, a structured reformer pilates exercise program was initiated under physiotherapist supervision, consisting of 2 sessions per week, each lasting 50 minutes, and continued regularly for 1 year. The exercise program included core, upper extremity ROM, upper and lower extremity strengthening exercises. Assessments were performed at baseline and after one year, including shoulder range of motion measured with a goniometer, upper extremity function evaluated using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire, pain assessed with the Visual Analog Scale (VAS), and body perception evaluated using the Body Image Scale (BIS). Results: The patient’s shoulder flexion improved from 100° to 180°, abduction from 90° to 180°, and internal rotation from 70° to 90°. The DASH score decreased from 22 to 11 points, pain during activity decreased from 4 to 1, night pain decreased from 5 to 2, and the BIS score increased from 162 to 189. Conclusion: In this case report, reformer pilates exercises performed under physiotherapist supervision demonstrated positive effects on shoulder ROM, upper extremity function, pain, and body image following breast cancer surgery. The effectiveness of long-term and regular reformer pilates exercises should be further investigated in individuals diagnosed with breast cancer.
