Abstract

Objective: In this study, we aimed to evaluate the localization of femoral fractures, associated secondary injuries, mechanism of trauma, and treatment methods in pediatric patients admitted to the emergency department. Materials and Methods: Medical records of patients under the age of 18 who presented to the emergency department with femoral fractures were retrospectively reviewed. Age, gender, mechanism of injury, fracture localization, treatment method, and the presence of secondary injury sites were recorded. Results: The mean age of the 82 patients diagnosed with femoral fractures was 8.79±4.69 years. Proximal femur fractures were observed in 10 patients, body fractures in 50 patients, and distal femur fractures in 27 patients. Both proximal and body femur fractures were observed in 3 patients, and both proximal and distal femur fractures were detected in 2 patients. Of the femoral fractures, 39 were treated surgically and 43 with splints. Among males, 64.10% underwent surgical treatment, whereas 51.16% of females were treated with splints (p=0.121). Secondary injuries were observed in 27 patients with femoral fractures. Multiple trauma was significantly associated with traffic accidents (p<0.05). Falls from the same level were more common in children under six years of age, whereas traffic accidents were more frequent in older children (p<0.001). Conclusion: Femoral fractures are most commonly caused by falls in the preschool period and by traffic accidents in children aged six years and older. The severity of trauma in the mechanism of injury increases the likelihood of secondary injuries in femoral fractures. Emergency physicians should assess the mechanism of injury and consider age-related secondary and multiple traumas when managing pediatric femoral fracture cases.
