Abstract

Hypothyroidism is a significant endocrine disorder resulting from insufficient thyroid hormones or their ineffectiveness in target tissues, and it can affect both the mother and the foetus during pregnancy. The similarity between the physiological changes seen in pregnancy and the symptoms of hypothyroidism complicates diagnosis and screening, thereby increasing the risk of complications. Thyroid dysfunction is among the most common endocrine problems during pregnancy after diabetes, and uncontrolled hypothyroidism is associated with adverse outcomes such as pregnancy-induced hypertension, pre eclampsia, anaemia, intrauterine growth restriction, and preterm birth. Oligohydramnios is characterised by a reduced amniotic fluid volume relative to gestational age and is often associated with increased perinatal morbidity, which has a multifactorial aetiology. In this case, a 28-year-old woman at 38 weeks of gestation, who had been diagnosed with primary hypothyroidism prior to pregnancy, was followed up with a diagnosis of oligohydramnios after an amniotic fluid index of 4.5 cm was measured. Levothyroxine treatment was adjusted according to changes in thyroid hormone levels during each trimester; the patient’s vital signs were stable, but pretibial oedema, decreased foetal movements, dyspnoea, sleep disturbance, loss of appetite, and concern for foetal harm were observed. Nursing diagnoses related to fluid balance, respiration, anxiety, sleep, nutrition, and lack of information were made according to the patient’s needs, and appropriate nursing interventions were planned and implemented. The pregnancy was terminated by caesarean section at 39 weeks, the mother and newborn were monitored and found to be stable, and education on home care and medication use was provided before discharge. This case demonstrates that failure to actively monitor pre-existing hypothyroidism during the pre-pregnancy period may predispose to obstetric complications such as oligohydramnios; it emphasises the critical role of a multidisciplinary approach and holistic nursing care in improving maternal and foetal outcomes. This case aims to present a concept map of oligohydramnios developing during pregnancy in a patient diagnosed with pre-pregnancy hypothyroidism.
