Abstract

The effects of natural disasters on women’s psychological well-being and reproductive health are considered a significant public health concern. This study was conducted to examine trauma levels and pregnancy avoidance intention among married women after a natural disaster in a Family Health Center affiliated with Adıyaman Province, one of the provinces affected by the Kahramanmaraş-centered earthquakes that occurred on February 6, 2023. A total of 327 datasets were analyzed. Data were collected using a Sociodemographic Information Form, the Post Earthquake Trauma Level Determination Scale (PETLDS), and the Pregnancy Avoidance Intention Scale (PAIS). The findings showed that the mean PETLDS score was 63.84 ± 15.14 among women aged 24 years and younger, while it was higher at 67.50 ± 16.65 among women with low income levels (p = 0.000). The mean total PAIS score was 2.52 ± 0.87, and the mean PETLDS score was 63.53 ± 15.86. A weak but statistically significant positive correlation was found between the total scores of the two scales (r = 0.168, p = 0.002), indicating that as post earthquake trauma levels increased, pregnancy avoidance intention also increased. The findings suggest that post earthquake trauma may influence women’s reproductive related decisions. The results also indicate that women with low income levels and those who experienced the loss of relatives or financial loss after the earthquake had higher trauma levels. In addition, certain sociodemographic, obstetric, and earthquake related characteristics such as age, income level, mode of delivery, and post earthquake losses were found to affect pregnancy avoidance intention. The results of the study were discussed in the literature to guide future research and indicate the need to strengthen psychosocial support and reproductive health services for women in primary health care settings after natural disasters.
