Abstract

Our aim in this study is to compare effects of sevoflurane and desflurane on haemodynamics and systemic oxygen consumption. 40 patients undergoing elective coronary artery bypass procedure were included in study. These patients were divided into three groups. We used 1 MAC sevoflurane in Group 1 (n=14) 1 MAC desflurane in Group 2 (n=13) and 0.5 MAC desflurane in Group 3 as inhalational agent after induction of anaesthesia until entrance cardiopulmonary bypass.. Haemodynamic measurements were recorded after entubation (T1), after surgical incision (T2), after sternotomy (T3) and before aortic canulation (T4). Mean arterial pressure was higher in all three groups at T2 period. Cardiac output was significantly higher in Group 2 at T2. We also observed that sevoflurane reduces mean pulmonary artery pressure (MPAP) more than desflurane. Total fentanyl use was not different between groups. Systemic oxygen consumption was not different between three groups. This study shows that in coronary artery patients who have normal functioning ventricules sevoflurane and desflurane displays similar haemodynamic changes, similar responses against surgical stimulation and systemic oxygen consumption. Although new inhalation anaesthetic desflurane shows comparable effects on hemodynamics and systemic oxygen consumption with sevoflurane, nevertheless sevoflurane seems to be a better choice especially in patients with right ventricle insufficiency and pulmonary hypertension.
