Abstract

Aim: The aim of this study was to compare intraoperative effects of hypertonic saline (HS, 3 % NaCl) in hypervolemic hemodilution versus 6 % hydroxyethyl starch (HES, mean molecular weight 130kDA/0.4) on hemodynamic and coagulation parameters in patients scheduled for total hip replacement surgery.Materials and Methods: Twenty patients scheduled to undergo total hip replacement with standard spino-epidural anesthesia were randomly allocated into two groups. Study solutions consisting of either HS % 3 or HES 6 % solutions in coded bags were delivered at a dose of 250 mL/m2 in 20 minutes. Vital signs and, laboratory parameters such as complete blood count, prothrombin time, activated partial thromboplastin time and fibrinogen, coagulation factors (II, V, VII, VIII, X) were assessed before and after study fluid loading, after completion of the surgery and 24 hours after the surgery. Results: The mean (±SD) amount of the study fluids infused were 382±135 mL HS and 459±67 mL HES. Hemoglobin concentrations and platelet counts decreased with time, mean blood pressures and heart rates remained stable before and after fluid loading (p
