Abstract

Purpose: To determine the activities of lymphocyte subgroups and natural killer (NK) cells in recurrent herpetic stromal keratitis (HSK). Material & Method: The study group consisted of 11 patients, including 5 male and 6 female diagnosed to have HSK with a mean age of 35.3 ± 6.2 years (range 24-44). The control group consisted of 10 healthy subjects including 4 female and 6 male with a mean age of 24.3 ± 8.0 years (range 16-30). Cytomegalovirus, toxoplasma and rubella antibodies were analyzed in the patient group and for delayed hypersensitivity skin test; purified protein derivation (PPD) was performed. Immunoglobulin (Ig) levels and subgroups were evaluated in both groups using ftephelometry. In the peripheral blood sample, lymphocyte subgroups were determined using FACSCalibur while NK cell activity was determined using anticandidal index (ACI). Mann Whitney U test was used for statistical analysis. Results: In herpetic group the findings revealed were cytomegalovirus IgG 100% (+), and IgM 100% (-), toxoplasma IgG 27.3% (+), rubella IgM 100% (-). PPD was found (+) in all patients. The IgG level was higher in the patient group than the control group (p=0.07) while IgA, IgM and IgG subgroups did not show any statistically significant difference between the groups. Although there was no statistically significant difference between the patient and control groups with regard to cell surface antigens CD3, CD4, CD19, CD20 and CD16 molecules; CD8+ cytotoxic T lymphocytes was found statistically higher (p=0.003) and therefore the CD4/CD8 ratio was found statistically lower in the patient group when compared to that of the control group (p=0.021). NK cell activity was significantly lower in the patient group (p=0.011). Conclusion: The variations observed in the peripheral blood lymphocyte subgroups and functions show that CD8+ T cells and NK cell activity are key factors in the natural immunity against recurrent herpetic stromal keratitis.
