Abstract

The preemptive analgesia, means to block pain by initiating a therapy prior to surgical attempt, prevents the intra-operative nociception during surgery and central responsiveness that occurred postoperatively. The aim of our study was to compare the postoperative analgesic effectiveness between Tramadol HCl and Lornoxicom (IV preoperatively) in patients undergoing lower abdominal surgery. After our hospital ethic committee’s approval, 80 ASA I patients, 18-60 years old, scheduling abdominal hysterectomy, were included in study. Patients were randomly divided into two groups (n=40). Group 1 received 8 mg lornoxicom IV prior to anesthesia and group 2 received 100 mg tramadol IV. Any additional analgesic drugs were not applied to the patients for one hour postoperatively. After the values of Visual Pain Scale were recorded, morphine HCl infusion with patient-controlled analgesia (PCA) was initiated. Following placement of PCA device, 1st, 2nd, 3rd, 4th, 8th, 12th, 18th and 24th hours patients’ SAP, DAP; MAP, HR and visual analogue scale values, first analgesia time and total analgesia time demands were checked and recorded.A difference was not observed between both groups in terms of SAP, DAP, VAS and HR values except the HR obtained 1st hour postoperatively. There were not significant differences their first analgesia time and total analgesia time demands between groups.Consequently, our results showed that lornoxicom which was administered as a preemptive analgesia for gynecologic operation was equal to tramadol. Thus, we consider that lornoxicom as a preemptive analgesic would be an alternative to tramadol.

Özet

Cerrahi girişim öncesinde tedaviye başlayarak ağrıyı önlemek anlamına gelen preemptif analjezi, cerrahi süresince intraoperatif nosisepsiyonu ve cerrahi hasar sonrası oluşan santral duyarlılığı önler. Çalışmamızın amacı, alt batın cerrahisi uygulanacak hastalarda operasyon öncesi uygulanan Tramadol HCl ile oksikam grubunun yeni üyesi Lornoksikam’ın operasyon sonrası analjezik etkinliğinin karşılaştırılmasıdır.Hastanemiz etik kurul onayı alındıktan sonra abdominal histerektomi uygulanacak, ASA1 sınıflamasına uyan 18-60 yaş arası 80 hasta çalışmaya alındı. Hastalar rastlantısal olarak iki gruba ayrıldı (n=40). Premedikasyon uygulanmayan hastalardan 1. gruba anestezi öncesi 8 mg lornoksikam ‹V, 2 gruba 100 mg tramadol ‹V uygulandı.
