Abstract

Introduction: Gynecological pathologies involving bones, breast, and cervix can be expected to occur due to chronic use of antipsychotic drugs. A possible relationship has been proposed between the elevation of prolactin (PRL) levels and the emergence of such pathologies due to antipsychotics. However, a number of studies comparing the occurrence rates of bone and gynecological pathologies during chronically PRL elevating and PRLnon-elevating antipsychotic use in regularly menstruating women, and the occurrence rates of those pathologies in a control group, have been inconclusive. Objective: The aim of the study was to examine the effects of using PRL elevating and PRL non-elevating antipsychotics on bone, breast, and cervix and to compare these findings with those of the control group. Method: One hundred thirty-six inpatients, chosen according to study inclusion and exclusion criteria, and who were receiving chronic antipsychotic treatment, were included as the study group and 140 women, who were similar to the study group in terms of weight and height, and were receiving no antipsychotic treatment, were selected as the control group. The patients were divided into two subgroups, those using PRL elevating antipsychotics (n = 42) and those using PRL non-elevating antipsychotics (n = 94). Prolactin levels, bone mineral density, and mammographic and cervical microscopic findings of all subjects were examined and compared. The Chi-square test, t test, Spearman’s correlation test, one-way analysis of variance (ANOVA) and regression analysis were used in the comparisons. Results: There were no significant differences with respect to age, height, and body mass index between the patient and control groups. PRL levels between the groups were significantly different. The average bone mineral density and osteoporosis rates were found to be similar in both groups. No significant differences in the incidence of radiological findings in terms of breast cancer, requiring further investigation, were detected between the two groups. There was no difference in incidence of abnormal microscopic cells, requiring further examination in terms of cervical cancer, between the groups. Discussion: Antipsychotic use did not affect the bone mineral density in regularly menstruating women, regardless of their PRL levels in this study population. Antipsychotic use did not affect the appearance rate of important mammography findings. In addition, antipsychotic use was determined not to affect the appearance rate of microscopic findings of the cervix which may be related to malignancy. According to these results, use of both PRL elevating and non-elevating antipsychotic drugs can be considered safe in terms of bone and gynecological malignancies in regularly menstruating female patients.
