Abstract

Objective: To determine diagnostic effectiveness of saline sonohysterography in abnormal uterine bleeding. Material and Method: Total of 70 pre- and postmenopausal women with complaints of abnormal uterine bleeding were recruited into study. After transvaginal ultrasonography, saline sonohysterography was used to determine intracavitary space occupying lesions. Then, hysterescopy was performed. Results: With transvaginal ultrasonography, 11 (%15.7) subjects was diagnosed as normal, 55 (%78.6) as irregular endometrium, 4 (%5.7) as myoma. With sonohysterography, 4 (%5.7) subjects were evaluated as normal uterus, 1 (%1.4) as irregular endometrium, 4 (%5.7) as myoma, 60 (%85.7) as polyp, 1 (%1.4) as atrophic endometrium. By hysterescopy, 5 (%7.1) subjects had normal uteruses, 4 (%5.7) had myoma, 60 (%85.7) had polyps, and 1 (1.4) had atrophic endometrium. In patients with pathologies, histopatologic evaluation was performed. Sensitivity of sonohysterography was % 100, specificity was % 80, positive predictive value was % 98.4, and negative predictive value was % 100. Conclusion: Sonohysterography is economic, and safe with high sensitivity in the evaluation of endometrial pathologies that can be easily used as an office procedure.

Bulgular: Transvaginal sonografi ile hastaların 11’i (%15.7) normal, 55’i (%78.6)düzensiz endometrium bulguları nedeniyle irregüler, 4’ü (%5.7) myom olarak değerlendirildi. Sonohisterografi uygulanan 70 hastanın 4’ünde (%5.7) normal uterus, 1’inde (%1.4) düzensiz endometrium, 4’ünde (%5.7) myom, 60’ında (%85.7) polip, 1’inde (%1.4) atrofik endometrium tespit edildi. Sonohisterografi ardından histeroskopi uygulanan hastaların 5’inde (%7.1) normal uterus, 4’ünde (%5.7) myom, 60’ında (%85.7) polip, 1’inde (1.4) atrofik endometrium saptandı. Patoloji saptanan hastalarda histopatolojik değerlendirme yapıldı. Sonohisterografinin sensitivitesi % 100, spesifitesi % 80, pozitif prediktif değeri % 98.4, negatif prediktif değeri % 100 olarak hesaplandı.Yorum: Sonohisterografi; endometrial patolojilerin tespitinde anestezi gerektirmeden ofis ortamında yapılabilecek, tanı değeri yüksek, ekonomik, uygulaması kolay ve güvenilir bir yöntemdir.
