Abstract

The perimenopause is a process that occurs in women beginning several years before the cessation of menses. The experience of perimenopause is variable in duration and symptom severity in women. Major gynecologic problems seen during the perimeııopause include abnormal uterine bleeding and vasomotor symptoms. Other common gynecologic problems relevant to perimenopausal women include vaginal mucosal atrophy and sexual dysfunction. The physicianpatient relationship is an effective tool to help guide a woman through the biological and life-cycle changes that are occurring. Symptoms can be controlled effectively with estrogen replacement therapy and some other medications but must be individualized to each patient's needs and risks. Attention must be paid to reliable contraception during this transition period although fertility is significantly decreased.
