Abstract

The aim of this study was to determine the risk factors, microorganisms and antibiotic susceptibility of nosocomial urinary tract infections in Clinics of Internal Medicine and Infection Diseases. The study included 118 patients 67 (56.8%) of whom were female and 51 (43.2%) male (mean age 48.64 ±18.9). Risk factors were as follows: 57.6% urinary catheter, 20% Diabetes mellitus, 10% malignancy and 10% urinary stone. The most frequent reason to carry urinary catheter was unconsciousness. Of isolated microorganisms 86 (72.8%) were Gram-negative bacteria, 20 (17%) were Gram-positive bacteria and 12 (10%) were Candida spp. The most frequently isolated Gram-negative bacteria were Escherichia coli (34%), Klebsiella pneumoniae (13.5%), Pseudomonas aeruginosa (%11), Enterobacter spp. (5.9%), Acinetobacter spp. (4.2%), Proteus spp.(4.2%). The most frequently isolated Gram-positive bacteria were Enterococcus faecalis (8.5%), Enterococcus faecium (4.2%), Staphylococcus aureus (3.4%), Staphylococcus epidermidis (0.8%). Of patients 62 (52.5%) had used antibiotic prior to urinary cultures. Of isolated bacteria 96.2% were resistant to two or more antibiotics.

Özet

Bu çalışma; iç Hastalıkları ve infeksiyon Hastalıkları servislerinde nozokomiyal üriner sistem infeksiyonlarından sorumlu predispozan faktörler, etkenler ve antibiyotik duyarlılıklarını saptamak amacıyla yapılmıştır. Çalışmaya 67'si (%56.8) kadın, 51'i (%43.2) erkek 118 hasta alındı (yaş ortalaması 48.64 ±18.9). Gram-negatif bakterilerin dağılımı sırasıyla; Escheri-chia coli %34, Klebsiella pneumoniae %13.5, Pseudomonas aerugiriosa %11, Enterobacter spp. %5.9, Acinetobacter spp. %4.2, Proteus spp.%4.2 idi. Gram-pozitif bakterilerin dağılımı ise; Enterococcus faecalis %8.5, Enterococcus faecium %4.2, Staphylococcus aureus % 3.4 ve Staphylococcus epidermidis %0.8 idi. Hastaların %52.5 önceden antibiyotik alıyordu, izole edilen mikroorganizmaların %96.2'si iki ve daha fazla antibiyotiğe dirençli idi.
