Abstract

Objective: The aim of this study is to compare lipoprotein (a) [Lp(a)] levels in patients with low and high risk unstable angina pectoris which is defined according to the cardiac troponin-I (Tn-I) levels and to investigate their relation with myocardial damage. Methods: From patients with chest pain; venous blood samples were collected for measuring serum Lp(a) and CRP (C-reactive protein) levels on admission and serum cTn-I levels 12 and 24 hours after admission. Fifty-nine patients with serum cTn-I levels 0.05). Conclusion: Increased Lp(a) levels and significant relation between Lp(a) and cTn-I levels support that Lp(a) can be a risk factor for plaque destabilization and thrombosis rather than severity of coronary artery disease in patients with high risk unstable angina. Furthermore, high levels of Lp(a) may be related with myocardial injury in patients with unstable angina.

Özet

Amaç: Bu çalışmanın amacı, kardiyak troponin-I (cTn-I) düzeyleri ile tanımlanan yüksek ve düşük riskli kararsız anginalı hastalarda lipoprotein(a) [Lp(a)] düzeylerini karşılaştırmak ve miyokard hasarı ile ilişkisini araştırmaktır. Yöntemler: Göğüs ağrısı yakınması ile başvuran hastalarda, başvuru sırasında Lp(a) ve C-reaktif protein (CRP) ölçümü için, başvurudan 12 ve 24 saat sonra da cTn-I ölçümü için venöz kan örnekleri alındı. Hastalar cTn-I değerleri 1.0 ng/ml'nin altındaki 59 hasta cTn-I negatif kararsız angina, cTn-I değerleri 1.0 ng/ml ve üzerindeki 53 hasta cTn-I pozitif kararsız angina olarak gruplandırıldı. Tüm hastalarda anjiyografik olarak koroner arter hastalığının şiddeti belirlendi. Bulgular: Başvurudaki Lp(a) düzeyleri, cTn-I pozitif grupta cTn-I negatif gruba göre anlamlı olarak daha yüksek bulundu (52.94±6.02 mg/dl ve 15.73±2.52 mg/dl, p0.05). Sonuç: Artmış Lp(a) değerleri ve Lp(a) ile cTn-I arasındaki anlamlı ilişki, yüksek riskli kararsız anginalı hastalarda Lp(a)'nın, koroner arter hastalığının şiddetinden ziyade, plak destabilizasyonu ve trombozis için risk faktörü olabileceği düşüncesini desteklemektedir. Ayrıca yüksek Lp(a) düzeyleri kararsız anginalı hastalarda miyokard hasarının gelişmesi ile ilişkili olabilir.
