Abstract

Objective: The study is performed to evaluate the effect of Pomeroy style tubal ligation on ovarian histopathology at 1th and 6th month. Institution: Firat Univesity Firat Medical Centre Obstetrics and Gynecologic Division, Elazig/TURKEY. Material And Methods: Adult 20 Winstar albino rats at eusrous phase divided into 4 groups randomly . Gl (n=5): Only laparatomy and 1 month later left oop-herectomy group. G2 (n=5): Left unilateral Pomeroy tubal ligation and 1 month later left oopherectomy group G3 (n=5): Only laparatomy and 6 months later left oop-herectomy group. G4 (n=5): Left unilateral Pomeroy tubal ligation and 6 months later left oopherectomy group Left ovary tissue fixed by 10% formaldehyde for histopathologic examination, embedded into paraffin block, sliced at 4 micrometer thickness and stained by hemotoxilen eosin. Primary, secondary and tertiary follicles counted under light microscope. Ovarian reserve determined by sum of all. Atretic follicles, corpus luteum, corpus albicans and total corpus counted. Regression of angiogenesis in corpous luteum evaluated. Fibrosis at ovarian stroma also evaluated. Ordinal scale done for regression of angiogenesis in corpus luteum and fibrosis at ovarian stroma (none=0p, present=1p, markedly present=2p). Ovarian follicle cysts counted and another nominal scale done for that (macros-copically none=0p, present=1p). Comparison done between Gl and G2 also between G3 and G4 (age compariable groups).Mann Witney U test used for ordinal data, X test used for nominal datas. P<0.05 accepted as statistically meaningful. Spearman relation analysis applied for parameters of ovary histology. (r,p,n). Results: Angiogenesis regression in corpus luteum was significantly lower, at G2 when compared with Gl (p<0.05, Mann Whitney U test). The other parameters were similiar. Angiogenesis regression in corpus luteum was significantly lower at G4 than G3 (p<0.05, Mann Whitney U test) and also atretic follicle count and ovarian stromal fibrosis development were significantly higher than G3 (p<0.05, Mann Whitney U test),the other parameters were found similiar (p>0.05, Mann Whitney U test). Macroscopic follicle cyst determined at one rat in G4 (p>0.05, X2 test). This follicle cyst was too large to make the other ovarian members atrophic. Conclusion: Left Pomeroy's style tubal ligation delay the angiogenesis regression in corpus luteum at I. and 6. months, additionally increases the atretic follicle and ovarian stromal fibrosis at 6. month. This ligation type leads macroscopic follicle cyst at ovaries. Pomeroy's style tubal ligation may have negatively effect on ovarian histopathology especially at long terms.

Özet

Amaç: Ratlarda pomeroy usulü tüp ligasyonunun 1. ve 6. ayda over histopatolojisi üzerine etkilerinin incelenmesi. Sol Over dokusu histopatolojik inceleme için %10’luk formaldehitle fikse edildi, parafin bloklara gömüldü, 4 mikrometre kalınlığında kesit alındı. Kesitler hematoksilen-eozin ile boyandı. Işık mikroskopisi altında incelenen preparatlarda primordial, primer, sekonder ve tersiyer foliküller sayıldı. Hepsi toplanarak over folikül rezervi saptandı. Pomeroy usulu tüp ligasyonu özellikle uzun dönemde over histopatolojisi üzerine olumsuz etki yapabilir.
