Abstract

Objective: To evaluate the effect of Nd:YAG laser iridotomy on the extent of pigment dispersion and intraocular pressure in Pigment Dispersion Syndrome. Methods: The study was carried out between December 2001 - March 2003 in the Department of Ophthalmology, Dokuz Eylül University, School of Medicine İzmir-Turkey. Eight eyes of four patients with Pigment Dispersion Syndrome diagnosed as having Ocular Hypertension or Pigmentary Glaucoma were included in the study. Three patients were female and one was male. All patients underwent routine ophthalmological examination including visual acuity, slit lamp examination, applanation tonometry, detailed fundus examination, Humprey automated perimetry and gonioscopy. Trabecular pigmentation was scored and Krukenberg spindle intensity was classified as gradel to 4. One eye of each patient was selected randomly for Nd:YAG laser iridotomy and the other eye was evaluated as the control. All patients were controlled at 1,2,6 and 12th months. Results: Trabecular pigmentation and Krukenberg spindle intensity tended to decrease during 12 months. While the initial trabecular pigmentation and Krukenberg spindle intensity was grade 3-4; after 12 months only one eye was grade 4 and the others were about grade 1-2. None of the patients showed progression in visual field defect. Intraocular pressures did not significiantly differ before and after the treatment. Conclusion: In Pigment Dispersion Syndrome, the mechanism leading to the deposition of pigment in the anterior segment structures (corneal endothelium and iridocorneal angle) results from rubbing of the posterior iris surface against the zonules, due to a posterior bowing of the concave iris root. Nd: YAG laser iridotomy reduces the pigment deposition.

Özet

Amaç: Pigmenter Glokom'da Nd:YAG lazer iridotominin Pigment Dispersiyon Sendromu üzerine etkilerini araştırmak. Gereç ve yöntem: Aralık 2001-Mart 2003 tarihleri arasında Dokuz Eylül Üniversitesi Göz Hastalıkları Anabilim Dalı Glokom Birimi'nde izlenen ve göz içi basınçları bir veya iki topikal antiglokomatöz ilaçla kontrol altına alınmış, Pigment Dispersiyon Sendromu olan Oküler Hipertansiyon veya Pigmenter Glokom tanısı almış 4 hastanın 8 gözü çalışma kapsamına alındı. Hastalardan l'i erkek 3'ü bayan idi.
