Abstract

Objective: Certain cytokines may contribute to the sequence of events that lead to meningeal inflammation in bacterial meningitis. The purpose of this study was to determine the levels of cytokine (tumor necrosis factor-a) in the cerebrospinal fluid (CSF) and blood of children with bacterial meningitis. Materials and methods: 30 patients (17 girl, 13 boy) in whom the diagnosis of acute pyogenic meningitis confirmed by analysis of the CSF, blood - CSF culture and latex aglutination test have been analysed. Associations between clinical findings and CSF parameters ( CSF protein, glucose, TNF , and blood TNF levels) before therapy and 48 hours later were investigated, and complications were compared between two groups. Results: The differences in the 48th hour TNF levels of CSF and blood between patients with and without dexametasone therapy were significant (p>0.05), while CSF protein and glucose were not significant (p<0.05). Only a (%7) patient developed complication in dexa (+) group while 4 (%28) patients in Dexa (-) group. Conclusion: Although the study number was small, our observation suggested that early dexametasone therapy might be beneficial for patients with acute bacterial meningitidis.

Özet

Amac: Bakteriyel menenjit tanısı alan 30 hastada adjuvan deksametazon tedavisinin beyin omurilik sıvısı ( BOS ) ve kan tumor nekroz faktor-alpha (TNF-a), BOS protein ve şeker düzeyleri üzerindeki etkisini görmek ve deksametazon kullanımının menenjit komplikasyonları ile ilişkisini saptamak amaçlanmıstır. Gereç ve Yöntem: Çalışmamıza BOS'un biokimyasal calışmaları, kültür ve Latex aglutinasyon testi ile akut bakteriyel menenjit tanısı alan 17 kız, 13 erkek toplam 30 hasta alındı. Randomize seçilen 15 hastaya deksametazon + antibiyotik verilirken, kalan 15 hastaya yalnız antibiyotik verildi. Tüm olgulardan 0. ve 48. saat BOS proteini, BOS şekeri, BOS ve kan TNF düzeyleri ölçüldü. Bulgular: Her iki grubun tedaviye yanıtları karşılaştrıldığında deksametazon + antibiyotik alan grupta deksametazon almayan gruba göre 48. saatte ölçülen BOS ve kan TNF düzeylerinde istatistiksel olarak anlamlı düşme saptanırken (16.58+0.61, 7.33+2.22; 13.34+19.26, 8.42+11.19 sırasıyla;p 0.05). Deksametazon alan grupta sadece 1 ( %7 ) hastada komplikasyon gelişirken, deksametazon almayan grupta 4 ( %28 ) hastada komplikasyon görüldü. Sonuç: Çalışma gruplarının tedavi sonrası sonuçlarının karşılaştırılmasında deksametazon kullanılan grupta yalnız antibiyotik kullanan gruba gore TNF değerlerinde anlamlı düşme saptanması ve daha az oranda komplikasyon görülmesi menenjit tedavisinde ek deksametazon tedavisinin yararlı olabileceğini göstermektedir.
