Abstract

Objective: Adnexal masses are encountered frequently and occasionally it is not possible to make a preoperative diagnosis even with the functional cysts. Preoperative diagnosis is inevitable to plan the proper surgical procedure and to maintain ovarian function with benign lesions. The aim of this study is to define the magnetic resonance imaging findings in various adnexal masses. Materials and methods: Thirty four patients, in whom adnexal masses were detected with pelvic ultrasound examination were included in the study. Avarage age of the patients was 43. Axial and sagital Tl weighted, axial T2 weighted and Tl weighted images with fat saturation and postgadolinium axial Tl weighted images with and without fat saturation were obtained in all patients. The localization, structural features ( contours, wall-thickness, scptations, solid mural nodules, papillary projections), signal intensity and contrast enhancement of the masses were evaluated. The presence of ascites and lympadenopathy was investigated. Final diagnosis was made with clinical follow-up in 9 eases and surgery in 25 cases. Results: The high soft tissue resolusion of magnetic resonance imaging enabled to display the structure ( wall-thickness, septations, solid mural nodules, multiloculations, cystic/solid components) of the lesions. Blood, fat, fibrous tissue, serous fluid and mucine could be defined and magnetic resonance imaging appearances of various ovarian masses could be defined. Conclusions: As more adnexal masses are evaluated with magnetic resonance imaging, the high soft tissue resolusion will make it possible to define the spesifie features of the masses and to prevent unnecessary surgical procedures.

Özet

Amaç: Adneksiyel kitleler sık karşılaşılan ve bazen fonksiyonel kistler de dahil olmak üzere preoperatif ayrımları yapılamayan lezyonlardır. Operasyon öncesinde olası tanının bilinmesi uygun cerrahi girişimin planlanmasını ve benign lezyonlarda over fonksiyonunun devamını sağlayabilir. Biz bu çalışmamızda değişik over lezyonlarının manyetik rezonans görünümlerinin tanımlanmasını amaçladık. Gereç ve Yöntem: Çalışmaya pelvik ultrasonografide (US) adneksiyel kitle saptanan toplam 34 olgu dahil edildi. Ortalama yaş 43 idi. Manyetik rezonans görüntüleme (MRG) incelemede tüm hastalarda aksiyel ve sagital Tl ağırlıklı, aksiyel T2 ve yağ baskılamalı Tl ağırlıklı ve gadolinium-DTPA enjeksiyonunu takiben, aksiyel Tl ağırlıklı ve yağ baskılamalı Tl ağırlıklı görüntüler alındı. MRG'de kitlelerin lokalizasyonu, büyüklüğü, morfolojik özellikleri (konturları, duvar kalınlıkları, septasyon, solid mural nodul ve papiller uzanımların varlığı), sinyal intensiteleri ve kontrast tutulumları değerlendirildi. Asit ve lenfadenopati varlığı araştırıldı. Dokuz hastada takip ile 25 hastada cerrahi girişim ile tanı koyuldu. Bulgular: Manyetik rezonans görüntülemenin yüksek yumuşak doku rezolüsyonu sayesinde lezyonların iç yapı özellikleri ( duvar kalınlığı, septasyon, solid mural nodul, tnultilokülasyon, kistik/ solid komponent) gösterilebilmiştir. Kan, yağ, fibröz doku, seröz sıvı ve müsinin ayırımı yapılarak değişik ovaryan kitlelerin görünümleri tanımlanmıştır. Sonuç: Adneksiyel kitlelere yönelik çalışmaların artması ve MRG' nin yumuşak dokuları değerlendirmedeki yüksek rezolüsyonu sayesinde adneksiyel lezyonların spesifik özellikleri belirlenebilir ve gereksiz radikal girişimler önlenebilir.
