Abstract

Objective: To compare the efficacy of intravitreal injections of aflibercept, ranibizumab, bevacizumab and dexamethasone implant and for the management of serous retinal detachment related to branch retinal vein occlusion. Material and Method: This retrospective study included 71 eyes of 71 patients with branch retinal vein occlusion and serous retinal detachment. Patients received one of the following treatments: Aflibercept (group 1, n=19), ranibizumab (group 2, n=19), 1.25mg/0.05mL bevacizumab (group 3, n=17), dexamethasone implant (group 4, n=16) and the 12-month treatment protocol in first 3 groups based on pro re nata protocol for all follow ups and followed by repeat injections based on clinical necessity. Group 4 received only a single dose of dexamethasone implant for 6 months, and then it was decided to recur according to the clinical situation. Best corrected visual acuity, central macular thickness, serous retinal detachment height and intraocular pressure measurements were done at baseline to 12 months of follow-up. Results: Mean number of injections per eye within twelve months were 4.63±1.94 (range, 1 to 8) in group 1, 4.52±2.43 (range, 1 to 12) in group 2, 3.29±1.21 in group 3 (range, 2 to 5), 2.31±0.60 (range, 1 to 3) in group 4, respectively. Branch retinal vein occlusion was significantly better in all groups in first 6 months but it became the worsened for group 4 in sixth months and after. Central macular thickness did not differ between groups in first 3 months, but it was significantly higher in first 3 groups at sixth month. serous retinal detachment height was significantly lower in group 1 in first 3 months. Intraocular pressure was significantly higher in group 4 in sixth and ninth months. Conclusion: Dexamethasone implant appears to be more advantageous in terms of branch retinal vein occlusion and serous retinal detachment height for the first 6 months in the treatment of serous retinal detachment secondary to branch retinal vein occlusion. However, at the end of the sixth month of treatment, anti-vascular endothelial growth factor drugs were more effective in sustaining increased branch retinal vein occlusion and decreased central macular thickness
