Abstract

Aim: The relationship between diabetes mellitus (DM) and coronary collateral circulation is not well known. A few limited study showed that coronary collateral circulation was poor in patients with DM. Also, it is not investigated that relationship between coronary collateral circulation and type of DM. The aim of this study was to evaluate the effects of insulin dependent diabetes mellitus (IDDM) and non-insulin dependent diabetes mellitus (NIDDM) on coronary collateral circulation compared to control group. Methods: One hundred patients with NIDDM (67 M, mean age: 59±8) and 100 patients with IDDM (60M, mean age:60±7 ) and 100 patients without DM (55 M, mean age: 58±9 ) as a control group were enrolled in this study. All the patients had at least one coronary artery narrowed more than 75%. Patients with three -vessel disease in whom all the vessels were narrowed more than 90% were excluded from the study. Also patients with valvular heart disease, formerly CABG operation and invasive coronary procedure, chronic obstructive lung disease, and systemic diseases (e.g.renal failure) were excluded. Coronary collateral circulation were graded according to the Rentrop scoring system. Results: There was no significant difference between three groups in regard to duration of the DM, baseline clinical and laboratory findings. The coronary collateral scoring was lower in patients with IDDM (1.51±1.06) than the NIDDM group(1.85±1.3, p<0.001) and control group (2.4±0.86, p<0.001), respectively). Also coronary collateral scoring was lower in patients with NIDDM than the control group (p<0.01). Conclusion: In all the patients with DM (whether IDDM or NIDDM), the coronary collateral scoring was significantly lower than the control group. On the other hand, in patients with IDDM the coronary collateral scoring was lower than the patients with NIDDM. We think that this effect is due to the insulin use in patients with IDDM.
