Abstract

Objective: To determine the value of different radiologic modalities in determining stone-free rates after extracorporeal shock wave lithotripsy (SWL) by further evaluating the patients considered to be stone-free according to plain abdominal x-rays, with ultrasonography (US) and spiral computed tomography (CT). Methods: Between March 2002 and November 2002, 53 patients with urolithiasis thought to be stone-free after SWL treatment were further evaluated with US and helical CT. The results were compared to those obtained with the conventional plain abdominal x-rays in terms of stone-free rates. Results: Among 53 patients considered stone-free according to the plain x-rays, residual stones were found in 5 cases (9.4%) with US and in 11 cases (20.7%) with helical CT. The mean size of the residual stones was 0.58 cm2 (range; 0.01-1.44 cm2). Residual stones were managed by additional SWL in one and ureteroscopic intervention in another patient. Conclusion: Although plain abdominal x-rays, cheap and practical, remain as the first choice of evaluation method after SWL treatment, helical CT appears to be superior in the detection of residual stones that could not be delineated in plain radiograms. As this finding may have impact on our clinical approach, we believe that routine use of helical BT can provide more accurate information infollowups after SWL.
