Abstract

Objective: To determine the value of far lateral prostate biopsies in addition to modified systematic sextant biopsy in the de-tection of prostate cancer. Methods: The study comprised 212 patients with a serum prostate specific antigen (PSA) value greater than 2.5 nglmL and/or nodular prostate on digital rectal examination between June 2002 and January 2003. Each patient underwent a 10 core biopsy protocol composed of a modified systematic sextant biopsy plus two far lateral peripheric zone biopsies on each side. Additional biopsies were performed from suspicious areas of the prostate. Results: Prostate cancer was detected in 23 (10.9%) of 212 patients. In 2 cases (8.6%) among biopsy positive patients the diagnoses were made based solely on far lateral peripheric zone biopsies. Conclusion: Although a definite conclusion can not be drawn due to the low cancer detection rate (10.9%) in our study, modified systematic sextant biopsy technique can be considered as almost an ideal method in the diagnosis of prostate cancer. The addition of far lateral biopsies may increase the cancer detection rate in patients with normal digital rectal examination and close to normal PSA elevations.
