Abstract

Introduction: Interest in the regulation of NGF (nerve growth factor) production in the urinary tract derives from its probable involvement in obstructive, inflammatory and developmental disorders. This study examines the NGF receptor immunreactivity in obstructive bladders. NGF plays an important role in the autonomic innervation of various organs in the body and bladder also. There are some hypotheses about the effects of NGF on bladder autonomic innervation and NGF accused about the changes seen in bladder during bladder outlet obstruction. We aimed to detect NGF levels in bladders of men with proven bladder outlet obstruction and compare the NGF levels after relief of obstruction. Materials and Methods: We performed physical examination, ufine analysis, urine culture (if necessary), total PSA levels in blood serum, urinary ultrasonography and urodynamic study to ali patients. in 20 patients with urodynamically confirmed outflow obstruction, detrusor biopsies were taken prior to and following 4 months after relief of obstruction. Cold cup biopsies of posterior wall of the urinary bladder were taken with Storz rjgid cystoscope 22 Fr. All the participants had given written informed consent before taking bladder biopsy. 10 control patients with urodynamically normal bladder without outflow obstruction undergoing a therapeutic procedure such as electrohyraulic lithotripsy of urinary stones included to study. The localization and amount of low-affinity nerve growth factor receptor (LNGFR) in the human urinary bladder were exanıined immunohistochemically using the mouse monoclonal antibody against human LNGFR in each specimen by one pathologist. The pathologist has not been aware of which group of specimen she was examining. Results: We detected NGF receptor immunreactivity in all specimens except one control patient. in all patients who showed NGF immune staining in epitlielium it was observed in surface epithelium. in study group there were patients who did not show epithelial staining but showed staining in nerve bundles. Al patients in the study group did have NGF immunreactivity. Only one patient in the control group we could not detect NGF immunreactivity. in patients with bladder outlet obstruction (BOO) preoperative and postoperative (after relief of obstruction) NGF immunreactivity were higher than control group (p>0.05). After relief of BOO NGF immunreactivity decreased with respect to preoperative values but it was not statistically significant. There were no correlation between IPSS, Qmax values and NGF immunreactivity (p>0.05). Conclusion: These findings provide that parenchymal cells in hypertrophied bladder and normal bladder can synthesize NGF and possibly other molecular messengers that act to alter the size and function of neurons in men. The temporal correlation of NGF receptor immunreactivity between obstructive group (preoperative), postoperative group and control group does not establish NGF as the causative factor for neurotrophic effects.
