Abstract

Objective: We compared laparoscopic transperitoneal and retroperitoneal approaches for decortication of Bosniak type I renal cysts with respect to safety and clinical results. Materials and methods: The study included 34 patients (15 males. 19 females; mean age 56 years; range 20 to 75 years) with symptomatic Bosniak type I renal cysts. Laparoscopic decortication was performed for 36 renal cysts using the transperitoneal (n=9, 25%) or retroperitoneal (n=27, 75%) approaches. For comparison of the two techniques, cyst laterality, size, localization, operation time, and previous renal or abdominal surgery were considered. The follow-up period was at least six months. Results: The mean cyst size was 7.2 cm (range 4 to 12 cm), being 6.3 cm and 7.5 cm for cysts removed by the transperitoneal and retroperitoneal approaches, respectively. Localizations were the lower pole in 33.3% (n=12), upper pole in 44.4% (n=16), and middle pole in 22.2% (n=8). The number of cysts involving the right and left kidneys was the same (n=18, 50%). The majority of the cysts (6/9, 66.7%) treated by the transperitoneal approach were in the upper pole. The mean operation time was 52 min, being 57.3 min for transperitoneal and 47.2 min for retroperitoneal approaches (p<0.05). The number of patients having previous abdominal surgery did not differ significantly between retroperitoneal (n=5) and transperitoneal (n=3) groups. Recurrence was seen in one patient in the retroperitoneal group. Conclusion: Our findings suggest that, except for the operation time, transperitoneal and retroperitoneal approaches have similar results in the laparoscopic treatment of Bosniak type I renal cysts, and that previous abdominal surgery, cyst size, and localization do not affect the success of the laparoscopic approach selected.

Özet

Amaç: Bosniak tip I böbrek kistlerinin tedavisinde laparoskopik transperitoneal ve retroperitoneal kist dekortikasyonu yöntemleri güvenlik ve klinik sonuçlar açısından karşılaştırıldı. Gereç ve yöntem: Çalışmaya semptomatik Bosniak tip I böbrek kisti olan 34 hasta (15 erkek, 19 kadın; ort. yaş 56; dağılım 20-75) alındı. Otuz altı kiste laparoskopik kist dekortikasyonu uygulandı. Dokuz kistte (%25) transperitoneal, 27 kistte (%75) retroperitoneal yaklaşım tercih edildi. İki tekniğin karşılaştırmasında, kist lateralitesi, büyüklüğü, yerleşimi, ameliyat süresi ve geçirilmiş cerrahi öyküsü değerlendirildi. Olguların en düşük takip süresi altı ay idi. Bul gu lar: Kistlerin ortalama büyüklüğü 7.2 cm (dağılım 4-12) idi. Transperitoneal ve retroperitoneal yaklaşım uygulanan kistlerin ortalama büyüklüğü sırasıyla 6.3 cm ve 7.5 cm idi. Yerleşim açısından 12 kist (%33.3) alt polde, 16 kist (%44.4) üst polde, sekiz kist (%22.2) orta polde idi. Kistlerin 18’i (%50) sağ, 18’i (%50) sol böbrek yerleşimliydi. So nuç: Bosniak tip I böbrek kistlerinin laparoskopik tedavisinde transperitoneal ve retroperitoneal yaklaşımlar arasında, ameliyat süresi dışında anlamlı fark yoktur. Daha önce geçirilmiş abdominal cerrahi, kist büyüklüğü ve yerleşimi seçilen laparoskopik yaklaşımın başarısını etkilememektedir.
