Abstract

In this study we investigated the efficacy and the clinical safety of the Cyclosporin-A in the treatment of rheumatoid arthritis. Twenty one patients diagnosed with rheumatoid arthritis according to the ARA criteria were enrolled to the study. Patients received low dose Cyclosporin-A for six months (maximum daily dose 4 mg/kg). Clinical and laboratory parameters were measured to asses the disease activity prior to and at the end of the treatment period. There were a significant improvement in the duration of morning stiffness, VAS pain score, Ritchie articular index. Lee functional index and serum CRP level. Bulber paralisy. total hearing loss, refractory hypertension, and anuria were the causes of cessation of the treatment in four patients. These all subsides after the cessation of the drug. We concluded that, Cyclosporin-A can be effective to control the activity of rheumatoid arthritis but a careful follow up is indicated.

Özet

Bu çalışmada, romatoid artrit tedavisinde düşük doz Siklosporin-A'nın etkinlik ve toleransı araştırıldı. Çalışmaya ARA kriterlerine göre romatoid artrit tanısı konmuş 21 hasta alındı.Hastalara altı ay süreyle düşük doz siklosporin-A verildi (maksimum günlük doz 4mg/kg). Tedavi başlangıç ve bitiminde klinik ve laboratuvar aktivite parametreleri istatistiksel olarak değerlendirildi. Altı aylık siklosporin-A uygulaması sonunda sabah tutukluğu süresi. VAS. Ritchie artiküler indeks, Lee fonksiyonel indeks, serum CRP değerlerinde anlamlı düzelme saptandı. Bir hasta bulber paralizi, bir hasta anüri, bir hasta total işitme kaybı, bir hasta kontrol edilemeyen hipertansiyon nedeniyle ilacı bırakmak zorunda kaldı. İlacın kesilmesinden bir süre sonra semptomlar düzeldi. Siklosporin-A'nın dikkatli takip ile uygulanmak koşulu ile romatoid artrit aktivitesini kontrol altına alabileceği sonucuna varıldı.
