Abstract

Objective: To evaluate the predictive value of second trimester cervical canal length measurement by transvaginal ultrasonography in preterm labour. Methods: The study group consisted of 120 patients without fetal anomalies, maternal medical diseases, multifetal pregnancies, history of previous preterm labour or cervical surgery. Maternal ages, last menstruel period, gravidity and parity were recorded. Cervical canal length measurement by transvaginal ultrasonography was done between 20 and 24 weeks of gestation and in order to estimate the risk of preterm labor sensitivity, specificity, positive and negative predictive values were calculated. With regard to parity, gestational age at birth and maternal ages, mean cervical length measurements were compared. Results: The mean gestational age at birth was 38 weeks. Nulliparity was observed in 54(45%) of the patients and 66(55%) were multiparous. Preterm delivery occurred in 14.2% (n=17) of the patients and 85.8% (n=103) delivered at term. The mean cervical canal measurement was found as 43.83 ± 7.66 mm. A statistically significant difference was found between mean cervical canal length measurements of preterm and term deliveries(p=0.012) but no statistically significant difference was found when patients were grouped with regard to parity. Preterm delivery was observed in 75% of the patients with a cervical canal length of <30 mm. Positive predictive value was 75% for a cutoff value of < 30mm and 100% for <25 mm. Conclusion: Second trimester cervical canal length measurement of <30mm performed by transvaginal ultrasonography is highly predictive of preterm labour.

Özet

Amaç: İkinci trimesterde transvaginal ultrasonografi servikal uzunluk ölçümünün preterm eylemdeki prediktif değerini araştırdık. Yöntem: İlk trimesterde başvuran, fetal anomalisi, maternal medikal hastalığı, çoğul gebeliği olmayan, servikal cerrahi uygulanmamış 120 hasta çalışma programına alındı. Maternal yaş, son adet tarihi, gravida, parite kaydedildi. Hastaların 54 (%45)’i nullipar, 66 (%55)’i multipardı.
