Abstract

Aim: Hyperlipidemia is one of the major risk factor associated with cardiovascular morbidity and mortality. Statins and newly introduced agent ezetimibe have been used for hyperlipidemia management. These drugs might be used alone or in combination. In addition, it has been shown that these drugs reduce hsCRP level and this inflammatory marker is associated with atherosclerosis. The goal of this study was to examine prospectively the effects of atorvastatin, rosuvastatin, atorvastatin plus ezetimibe, and rosuvastatin plus ezetimibe on total cholesterol, LDL-cholesterol, HDL-cholesterol, and triglyceride levels and hsCRP level in patients with hyperlipidemia. Material and Methods: Forty seven patients received atorvastatin 10-20-40 mg (group 1), 45 received rosuvastatin 10-20 mg (group 2), 30 received atorvastatin plus ezetimibe 10-20 + 10 mg (group 3), 24 received rosuvastatin plus ezetimibe 10-20 + 10 mg (group 4) for 4 weeks in a prospective manner. The mean age of the study population was 58 ± 11 years. Sixty percent of patients were male. Sixty-six percent of patients had hypertension, 32% had diabetes, 23% had family history for coronary artery disease, 23% had smoking habit, and 32% had coronary artery disease. Results: Groups were well matched concerning baseline demographic and laboratory characteristics. At the end of the four weeks atorvastatin reduced TC by 21%, LDL-C by 30%, TG by 22% and increased HDL-C by 13%. Rosuvastatin reduced TC by 27%, LDL-C by 41%, TG by 26% and increased HDL-C by 25%. Atorvastatin plus ezetimibe reduced TC by 33%, LDL-C by 45%, TG by 31% and increased HDLC by 12%. Rosuvastatin plus ezetimibe reduced TC by 31%, LDL-C by 49%, TG by 30% and increased HDL-C by 22%. There was no statistical significance associated with increased HDL-C and decreased TG levels between groups (mean % change from baseline to week 4). However, atorvastatin plus ezetimibe and rosuvastatin plus ezetimibe significantly reduced TC compared to atorvastatin (p=0.008 and 0.013, respectively). In addition, there was a significant association concerning LDL-C mean % change from baseline to week 4 between groups. At the end of the four weeks hsCRP was reduced by 11% in Group 1, 23% in Group 2, 38% in Group 3, and 42% in group 4. A statistical significance was found between Group 1 and Group 3 (p=0.041), Group 1 and Group 4 (p=0.033), Group 2 and Group 3 (p=0.036), Group 2 and Group 4 (p=0.014). However, there was no significant relationship between Group 1 and Group 2, and Group 3 and Group 4. Only one patient had increased ALT or AST > 3 times the upper limit of normal as an adverse effect (7‰). Conclusion: Rosuvastatin alone and in combination with ezetimibe improved the lipid profile, especially the LDL-C, of patients with hyperlipidemia compared to atorvastatin alone. However, both statins in combination with ezetimibe almost equally decrease LDL-C. Rosuvastatin plus ezetimibe was also more efficacious in decreasing the LDL-C level. Statins decreased hsCRP in patients with hyperlipidemia. Addition of ezetimibe to statins was more efficacious in decreasing hsCRP level compared to statins alone.
