Abstract

AIM: We evaluated the effect of desmopressin on blood loss and transfusion requirement in the patients taking aspirin before the coronary artery bypass grafting surgery. METHOD: Aspirin was given to thirty patients before coronary artery bypass graft (CABG) surgery. The patients were separated into two groups of fifteen patients in each other. In the first of patients, plasebo was given. In the second group of patients, desmopressin acetat (0.3 microgram/kg) was given. Desmopressin acetat (DDAVP, I-desamino-D-arginin vasopressin) was infused in 100 ml of physiological serum during twenty minutes. RESULTS: The amount of mediastinal drainage was mesasured. Biochemical, haematologic, bleeding coagulation tests were applied and evaluated all together. The amount of mediastinal drainage was found significantly decreased in the second group of patients. In the first group, drainage was 1048±91.97 cc and in the second group 690±59.34 cc (p=0.003). There was no statistically significant change on ECG recording, early and late postoperative infarction rate and cerebrovascular event between the two groups. CONCLUSION: We can conclude that DDAVP could be used safely in CABG surgery. Because of the significantly decreased total amount of blood loss in the second group, the need of the whole blood and blood products transfusion was minimal.

Özet

AMAÇ: Koroner arter bypass cerrahisi öncesinde aspirin alan hastalarda desmopressinin kan kaybı ve transfüzyon ihtiyacına olan etkisini incelemektir. YÖNTEM: Koroner arter bypass greft (KABG) cerrahisi öncesinde aspirin alan 30 hasta, 15 'er kişilik iki grup halinde çalışma kapsamına alındı. Kontrol grubunu oluşturan birinci grup olgulara plasebo, ikinci gruptaki olgulara protamin sülfat verildikten sonra 0.3 mikrogram/kg desmopressin asetat (DDAVP, I-desamino-8-D-arginine vasopressin) 100 ml serum fizyolojik içinde 20 dakikada verildi. 1. Grupta drenaj miktarı 1048±91.97 cc, II. Grupta 690.67±59.34 cc olarak gerçekleşti (p=0.003). Her iki gruptaki olgular karşılaştırıldığında, olgularda anlamlı EKG değişikliği, erken miyokard infarktüsü, serebrovasküler olay, tromboemboli yönünden fark saptanmadı. Desmopressin'in potent antidiüretik etkisi yönünden belirgin elektrolit değişikliği saptanmadı. SONUÇ: Desmopressin asetat koroner arter bypass cerrahisi öncesinde aspirin alan ve kardiyopulmoner bypass uygulanan hastalarda cerrahi olmayan nedenlere bağlı kanamaları, kan ve kan ürünleri transfüzyon ihtiyacını azaltmada güvenle kullanılabileceği kanısındayız.
