Abstract

AIM : To test if coronary ischemiea occurs with treadmill exercise testing in patients with coronary artery ectasia but without significant stenosis. METARIAL AND METHODS : Patients noted to have coronary artery ectasia without significant stenosis in the catheterization laboratory between January 1998 and April 2000 formed the study group (17 men 4 women, mean age 55.1 ± 7.2). If a treadmill exercise testing was not performed before coronary angiogram it was performed after the angiogram. To be included in the study group patients needed to have no significant ECG abnormalities (bundle branch block, intraventricular conduction defect, ST-T abnormalities), no wall motion abnormalities and no myocardial infarction anemnesis. Control group was formed from patients with purely normal coronary arteries who have not performed a treadmill test before coronary angiogram.Their age, sex and coronary artery disease risk factor profile was similar with the ectasia group (16 men, 3 women age 53.2 ±8.4). RESULTS : The ectasia group and the study group were both similar interms of age, sex and coronary artery disease risk factor profile. Treadmill test results were positive in 12 of 21 patients in the ectasia group and, 2 of 19 in the control group (p=0.006). CONCLUSION: Ischemia can occur in patients with coronary artery ectasia even without significant stenosis.

Özet

AMAÇ : Tıkayıcı lezyonu olmayan, koroner arter ektazisi olan hastalarda koroner iskemi gelişip gelişmediğinin efor testi ile incelenmesi. GEREÇ VE YÖNTEM : Ankara Üniversitesi Kalp Merkezinde Ocak 1998-Nisan 2000 tarihleri arasında koroner anjiyografi neticesinde koroner arter ektazisi olan ancak tıkayıcı lezyonu olmayan (< % 50) 17 erkek, 4 kadın (ortalama yaş 55.1 ± 7.1) hasta çalışma grubunu oluşturdu. Koroner anjiyografisinde ektazi saptanan, duvar hareket bozukluğu bulunmayan, myokard infarktüsü anamnezi olmayan, ve EKG 'sinde dal bloğu, intraventriküler ileti gecikmesi, önemli ST-T değişikliği olmayan hastalar çalışmaya alındı. Koroner anjiyografi öncesi efor testleri yapılmamış ise hastalara efor testi yapıldı. Kontrol grubu da koronerleri tamamen normal bulunan ve koroner anjiyo işlemi öncesi efor testi incelemesi yapılmamış yaş, cinsiyet, koroner arter hastalığı risk profili benzer hastalara treadmill standart Bruce protokolu ile efor testi yapılarak oluşturuldu (16 erkek, 3 kadın yaş 53.2 ±8.4). BULGULAR : Ektazi ve kontrol grubunun yaşları ve koroner arter hastalığı yönünden risk profili benzer özellikteydiler. Efor testlerinin incelenmesi sonucunda ektazi grubunda 12 hastanın efor testi ST segment çökmesi nedeni ile testi pozitif olarak yorumlandı.Kontrol grubunda 2 hastada efor testi pozitifdi (p=0.006). SONUÇ : Tıkayıcı lezyonu olmasa da koroner arter ektazisi olan hastalarda koroner iskemi görülebilir.
