Abstract

AIM: The purpose of this investigation was to assess a correlation betwen coronary risk factors, collateral blood flow and left ventricular wall motions in patients who hadn't myocardial infarction inspite the presence of total occluded coronary vessel and protected left ventricul function. MATERIALS AND METHOD: We prospectively studied 83 patients with one total occluded vessel and < or =50 % diameter stenosis in at least one vessel documented by quantitative coronary angiography. Patients were divided into 2 groups. In group I (n=43) left ventricule wall motions were normal. And at least one segment abnormality was present in the other group (Group II, n=40). The TIMI Frame Count is used to determine coronary collateral blood flow measurements. The effect of the presence and duration of angina and other risk factors like age, smoking, sex, diabetes mellitus, hypertension and family history on the left ventricule wall motions was also investigated. RESULTS: There were no significant difference in Frame Count grades among patients with grade I collateral system. However, among patients with grade II and grade III collateral flow, Frame Count grades were significantly lower in group I compared with group II (P>0.05). No correlation existed between the localisation of the lesion, the presence of collateral flow and left ventricule wall motions in patients with total occluded vessels (P>0.05). The presence and duration of angina didn't effect the collateral grades and left ventricule wall motions. The effects of coronary risk factors on left ventricule wall motions were not significant (P>0.05). CONCLUSION: Patients with angiographically documented collateral flow rate protects left ventricule wall motions. Left ventricule function was more protected in patients with high collateral blood flow rate.
