Abstract

The purpose of the study was to evaluate alterations of the thrombophilic and fibrinolytic activation markers in patients with atrial fibrillation (AF). MATERIAL AND METHODS: Fifty eight patients with chronic AF were studied (34 women and 24 men; aged 65 ± 9,9). 26 patients had rheumatic heart disease and 32 patients had nonrheumatic atrial fibrillation. Control group was composed of 30 healty subjects. Plasma levels of antithrombin III, tissue plasminogen activator (TPA), plasminogen activator inhibitor (PAI-1), D-Dimer, and fibrinogen were measured in plasma with chronic AF. Levels were compared with those found in the plasma of control group. Transthorasic echocardiographic studies were done all patients and control group. RESULTS: Patients with AF had higher plasma levels of antithrombin III (p=0.021), TPA (p=0.015), D-Dimer (p=0.026) and fibrinogen (p=0.0001) than control. There were a positive correlation between left atrial diameter and plasma fibrinogen (r=0.612; p=0.001). There was no significant difference in plasma PAI-1 (p=0.79)). There were no differences in wiew of hematologic parameters between rheumatic and nonrheumatic atrial flbrillation (p=0.65). CONCLUSION: Patient with atrial fibrillation in addition to their hypercoagulable state have also been found to have impaired flbrinolytic function. Measurement of hematologic parameters may complement clinical and echocardiographic risk stratification for stroke and thromboembolism in atrial fibrillation.
