Abstract

Aim: We aimed to evaluate left atrial (LA) volume and mechanical functions before and six months after the procedure using realtime three-dimensional echocardiography (RT3DE) in patients who underwent percutaneous mitral balloon valvuloplasty (PMBV) due to mitral stenosis (MS). Material and Method: Thirty-six patients aged 24 to 58 years who underwent PMBV by the Inoue technique were included in the study. All patients included in the study underwent two-dimensional (2DE) and real-time three-dimensional echocardiography (RT3DE) before and six months after PMBV. Results: There were no significant differences in left ventricular, LA dimensions and ejection fraction assessed by 2DE between before and 6 months after PMBV. Mean pulmonary artery pressure, mitral valve area, peak and mean pressure gradients across mitral valve were decreased significantly 6 months after PMBV compared with those before PMBV (26.2±4.8 versus 10.4±2.1, p<0.0001; 1.12±0.2 vs 1.9±2.6, p<0.0001; 18±2.2 vs 8.2±4.7, p<0.0001; 12±4.3 vs 6±2.1, p<0.0001, respectively). While there were significant decreases in LA maximum volume and volume before atrial contraction months after PMBV assessed by RT3DE, decrease in LA minimum volume was not significant (71.52±13 vs 68.6±13.8, p<0.0001; 41.5±7 vs 38.9±7, p<0.0001; 28.7±6.7 vs 28±6, p=0.2, respectively). Although total stroke volume, total emptying fraction, active stroke volume and active emptying fraction were significantly decreased six months after PMBV, passive emptying fraction was significantly increased (43.5±10 vs 39.8±10, p<0.0001; 60.5±6.5 vs 57.7±7.7, p=0.003; 13.5±4 vs 10.2±4.4, p<0.0001; 32.7±9 vs 26.5±11, p=0.002; 41±9 vs 42.3±8.4, p=0.01, respectively). Conclusion: We showed that there is a reversible remodelling in LA after PMBV demostrated by RT3DE and there could be some improvements in LA volume and mechanical functions.
